By signing below, we acknowledge and agree to the following:

1. We are the parent(s)/guardian(s) of the student listed above and are current Christ Church School families in the current 4th grade class.
2. We are committing to our child’s enrollment in the founding 6th grade class of Christ Church School’s middle school.
3. We understand that, to participate in the Founding Class Commitment Program, this signed form and a non-refundable $1,000 deposit must be submitted no later than June 4, 2026.
4. We understand that, upon timely submission of this signed form and deposit, tuition for our child will be frozen at $19,500 per year for 6th, 7th, and 8th grades, provided our child remains continuously enrolled at Christ Church School.
5. We understand that if our child withdraws, does not enroll, or does not remain continuously enrolled at Christ Church School, the tuition freeze will no longer apply.
6. We understand that if we later withdraw after submitting this form and deposit, the $1,000 deposit will be forfeited, and our child’s place in the founding class will no longer be guaranteed.
